Physical restraints restrict a person's freedom of movement. Raised side rails on a bed physically limit the resident’s ability to move or get out of bed, which is why they’re considered a physical restraint. The other options aren’t restraints in the same way: medication used to control behavior is a chemical restraint, loosening clothing around the neck is unsafe and not a restraint, and calling for help is a response rather than a restraint. Restraints should only be used when truly necessary, with an order, and with ongoing monitoring, after trying less restrictive options first.
